What is Care Management?

Care Management provides client advocacy, assistance in facilitating communication with providers, and identification of services and resources. A Care Manager may recommend locations of care for the client and communicate with other allied professionals
regarding the client’s needs.

On-Going Care Management

  • On-Going Support & Supervision
  • Serve as liaison between clients, their families, and community resources
  • Train, orient, supervise and support in-home caregivers and other staff
