What is Care Management?

Care Management provides client advocacy, assistance in facilitating communication with providers, and identification of services and resources. A Care Manager may recommend locations of care for the client and communicate with other allied professionals
regarding the client’s needs.


Who is a Care Manager?

Care Managers are professionals that serve as consultants to seniors and their families. These experienced professionals assist the senior and their family with issues associated with long-term care.


What kinds of services are included in care management?

On-Going Care Management

  • On-Going Support & Supervision
  • Serve as liaison between clients, their families, and community resources
  • Train, orient, supervise and support in-home caregivers and other staff
  • Crisis Intervention
  • Medicare, health insurance and long-term care insurance advocate
  • Serve as a liaison with medical and other professionals
  • Social Interaction
  • Referral to Other Professionals

On-Site Home Management

  • Establishing home safety and emergency preparedness
  • Supervision of Home Modification, Repair and Maintenance

Financial Management

  • Pay Bills
  • Sort and file incoming mail
  • Balance checkbooks and maintain organization of bank records
  • Organize tax documents
  • Prepare and deliver bank deposits
  • Review medical insurance papers and verify proper processing of claims

Long-Term Care Management

  • Long Term Placement in Continuing Care Retirement Communities, Assisted Living Facilities or Nursing Homes
  • Work with a Senior Move Manager to assist in transition from home to long-term care community
  • Work with a Senior Real Estate Specialist to assist in selling home, if needed
  • Advocacy in Long Term Facilities
  • Assistance with Medicaid Applications for Long Term Care Placement
